At Avaera Health, we are dedicated to helping older adults stay healthier, longer through convenient preventive care. Our team partners with retirement communities, senior living facilities, and local organizations to provide Medicare Annual Wellness Visits at no cost to eligible patients.
Preventive care is one of the most powerful tools for maintaining independence and quality of life. By identifying potential health risks early, reviewing medications, and helping patients create personalized prevention plans, we empower seniors to take an active role in their health.

Meet the team
Founded by physicians who care deeply about preventive health
Avaera was founded by two physicians who share a common belief: preventive care should be easy to access, especially for older adults.
Dr. Jill Berger and Dr. Delali Sessinou created the practice with a simple goal — to help Medicare patients stay healthier by making Annual Wellness Visits more convenient and accessible.
During their years in clinical practice, they saw that many seniors were missing important preventive care simply because it was difficult to schedule appointments, travel to clinics, or navigate the healthcare system. Yet these visits can play a critical role in identifying health risks early, reviewing medications, and helping patients build a plan to maintain their independence and quality of life.
That insight led them to a different approach: bring preventive care directly to where seniors live. By partnering with retirement communities and senior living centers, they make it easy for Medicare beneficiaries to receive their Annual Wellness Visit in a comfortable and familiar environment.
Today, their focus remains the same — providing thoughtful, patient-centered care that helps seniors stay healthy, informed, and supported.
